Leaving the catholic church?

I was christened as a catholic but stopped any religious practise when I was around 12-13. Whenever I had to put "religion" in a form I would always say "none". However I never formally left the church and am wondering if I need to do this to feel free from any affiliation with it, in other words can someone still count me as a catholic even though I am not one anymore.
